FAQs
Our team members have worked with many of the world’s greatest athletes and are experts in sports physical therapy.
At CHP, we believe in taking the best care of you so that you can get out of pain and back doing what you love, faster and in less visits. Unlike standard physical therapy clinics, you will receive one on one care with a highly skilled physical therapist for the entirety of your sessions and your treatment plan will be customized to your needs and goals. This will dictate the frequency and duration of your plan. We believe in teaching you strategies that you can use independently so that you do not become dependent on us for long term results. This is vastly different from most PT clinics where you may be one of many patients being seen at one time, multiple times a week, with limited opportunities to learn what you can do on your own.
We believe that for profit health insurance shouldn’t dictate your results by determining what is “medically necessary” for you. Insurance companies do what is in their best interest, not YOURS! We are Out of Network with health insurance which makes it possible for you to get better results, in less visits. At CHP, our therapists determine what YOU need to achieve your goals and unlike in-network clinics, we refuse to allow our therapists to treat 2-3 patients at the same. You will always work one-on-one with your PT for the amount of time required to reach your goals.
Payment is due at the time of service but we will provide you with invoices that you can use to submit to your health insurance provider for out of network reimbursement. You can also use your flexible spending account or health savings account if you have one. The investment for a one time evaluation is $250, but your Doctor of Physical Therapy will recommend a treatment package that is the best fit for you and is discounted overall. Sessions (including the evaluation) in a treatment package average between $190 and $200 per session.
Your first visit is your initial evaluation. As with all visits to CHP, this session will be one-on-one with your physical therapist for the duration of the session. Unlike other physical therapy clinics, we will never split your time with other patients. In this one hour session we will determine the root cause of your pain or injury and recommend the treatment package necessary to get you the results you desire.
Please wear something that you feel comfortable moving in, such as workout clothes and your preferred method of payment. Don’t forget to login to your patient portal prior to your visit to fill out your new patient forms!
Yes! At CHP we believe that healthcare must be a collaborative effort. Every provider that you work with can have a positive impact on your health and recovery. We believe that you will greatly benefit when we all are working together to help you.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
